Does Medicare Cover Ozempic or Wegovy? — 2026

Ozempic and Wegovy are both semaglutide medications made by Novo Nordisk, but they have different FDA approvals and very different Medicare coverage rules.

Ozempic: Covered for Diabetes

Ozempic is FDA-approved for type 2 diabetes. Medicare Part D plans cover Ozempic when prescribed for blood sugar management. It is typically placed on Tier 3 or Tier 4, with copays ranging from $30 to $200+ per month depending on your plan. With the 2026 $2,000 annual out-of-pocket cap, your total drug spending for the year is limited regardless of how expensive your medications are.

Wegovy: Weight Loss Coverage Limited

Wegovy is FDA-approved for chronic weight management. Historically, Medicare has excluded weight loss drugs from Part D coverage. Legislative efforts to change this exclusion continue. As of 2026, check with your specific plan for the latest coverage status, as this area is actively evolving. Some Medicare Advantage plans may offer supplemental benefits that partially cover weight management programs or medications.

Important Distinction

Ozempic and Wegovy contain the same active ingredient (semaglutide) but at different doses and with different FDA indications. A doctor prescribing semaglutide for diabetes can use Ozempic and it will be covered by Part D. The same drug prescribed specifically for weight loss (Wegovy) faces coverage restrictions.

Cost-Reduction Strategies

Choose a Part D plan where semaglutide has favorable formulary placement. Use the Medicare Prescription Payment Plan to spread costs monthly. Ask about step therapy requirements — some plans require trying metformin or insulin first. Once you hit the $2,000 annual cap, all covered drugs are free for the rest of the year.
