Gather your information: List of current doctors and specialists with addresses. List of all prescription medications with dosages and frequency. Preferred pharmacy name and location. Current Medicare card (Original Medicare, Advantage, or both). Any upcoming scheduled procedures or specialist referrals. Income information if you may qualify for Extra Help or Medicaid.
Questions to Answer
Coverage needs: Are your current doctors accepting your plan next year? Has your plan's formulary changed for any of your medications? Has your plan's premium, copay, or deductible changed? Do you need dental, vision, or hearing coverage? Do you travel or spend time outside Utah? Have you had any major health changes this year?

Part B premium: $185/month. Part B deductible: $257/year. Part A hospital deductible: $1,676. Part D annual out-of-pocket cap: $2,000. Insulin cap: $35/month. Annual Enrollment Period: October 15 – December 7.
