How IRMAA works

Social Security looks back at your tax return from two years ago to decide whether you owe IRMAA today. It uses your modified adjusted gross income โ€” your adjusted gross income plus any tax-exempt interest. If you're above the first threshold, a surcharge is added to both your Part B premium and your Part D premium, in the tiers shown above.

Because of the two-year lookback, a big one-time income event (selling a home, a Roth conversion, capital gains) can push you into IRMAA even if your current income is lower. And if a life-changing event โ€” retirement, marriage, divorce, or the death of a spouse โ€” has reduced your income, you can appeal with Form SSA-44 and often have the surcharge reduced or removed.
